The type of personal information we collect  

We only collect data relevant to your therapy which will be password protected and encrypted drive or cloud-based server and paperwork will be stored in a locked cabinet. The information that we collect includes but is not limited to the below. 

  • Name  
  • Contact details 
  • Date of birth 
  • Medication details 
  • GP details  
  • Circumstances around why you require therapy 

How we get the personal information and why we have it 

The information we collect is the personal data you provide us. We require this information to create tailored therapy sessions. Contact information and addresses are required in the event of change in circumstances for us to either contact a medical professional or emergency services should you inform us that you may be at risk outside of a session to the safety of yourself or others. We do not collect any personal data from you that we do not need to oversee the therapeutic service with you. 

Under the UK General Data Protection Regulation (UK GDPR), the lawful bases we rely on for processing this information are: 

(a) Your consent. You are able to remove your consent at any time. You can do this by contacting tara@taraphillipshypnotherapy.com 

(b) We have a contractual obligation. 

(c) We have a legal obligation. 

(d) We have a vital interest. 

(e) We need it to perform a public task. 

(f) We have a legitimate interest. 

How long we keep your data 

We are required by our governing association membership to keep records of our clients for adults for 6 years from the end of their therapy. For clients under the age of 18, we are required by GDPR law to keep records until they turn 21 or for 6 years from the end of their therapy (whichever is greater). This includes both information about why you are seeking therapy and the notes taken during your sessions. 

Your data protection rights 

Under data protection law, you have rights including: 

Your right of access – You have the right to ask us for copies of your personal information.  

Your right to rectification – You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.  

Your right to erasure – You have the right to ask us to erase your personal information in certain circumstances.  

Your right to restriction of processing – You have the right to ask us to restrict the processing of your personal information in certain circumstances.  

Your right to object to processing – You have the the right to object to the processing of your personal information in certain circumstances. 

Your right to data portability – You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances. 

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.

How to complain 

If at any point you believe the information we process on you is incorrect, you can request to see this information and have it corrected or deleted.
