In order to provide safe and effective hypnotherapy and psychotherapy services, the following information may be collected:
– Your name and contact details
– Which town you are travelling from
– Relevant medical or health information
– GP details
– Brief session notes
– Information relating to your therapy goals
– mail, text or appointment communications
Only information necessary for therapy, safeguarding, communication, and legal obligations is collected.


LAWFUL BASIS FOR PROCESSING INFORMATION
Personal information is processed under the lawful basis of Legitimate Interests in order to provide therapeutic services safely and appropriately.
Your information is never sold or shared for marketing purposes.
Where consent is required, this will normally be discussed with you first. However, information may be shared without consent where there is a safeguarding concern, serious risk of harm, legal obligation, or where disclosure is otherwise required by law.


WHY IS THIS INFORMATION COLLECTED?
The information collected helps to:

  • Monitor progress appropriately
  • Ensure therapy remains safe and suitable
  • Manage appointments and communication
  • Meet professional and legal obligations
  • Unidentifiable information may occasionally contribute towards professional research relating to Solution Focused Hypnotherapy.


HOW IS MY INFORMATION STORED?
Reasonable steps are taken to keep your information secure. This includes:

  • Locked storage for paper records
  • Password protected computers and devices
  • Password protected email accounts
  • Secure access to client files and appointment systems
    Only Tara Phillips has access to identifiable client records unless legally required otherwise.

HOW LONG IS MY INFORMATION KEPT?
Client records are typically retained for 8 years following the end of therapy in line with professional indemnity insurance requirements and industry guidance.
For children, records are normally retained until their 25th birthday. Where therapy ends at age 17, records may be retained until age 26.
Records are securely destroyed once the retention period has expired.


CAN I ACCESS OR REQUEST DELETION OF MY INFORMATION?
Under UK GDPR you have the right to request access to the personal information held about you and to request deletion of information where appropriate.
Requests should be made in writing. Reasonable and proportionate searches will be carried out in line with UK GDPR. Identity may need to be confirmed before information is released or deleted.


IS WHAT I DISCUSS CONFIDENTIAL?
Everything discussed during sessions is treated confidentially.
Occasionally, elements of sessions may be discussed anonymously within professional supervision to ensure safe and effective practice.
Information may also be shared where there is a safeguarding concern, legal obligation, or court order. Where possible, this would normally be discussed with you first.


WHAT IF WE SEE EACH OTHER OUTSIDE OF SESSIONS?
Confidentiality will always be respected. For this reason, Tara Phillips may not initiate conversation outside of sessions unless you choose to do so first.


DATA PROTECTION COMPLAINTS
If you have concerns about how your personal information has been collected, stored, or used, you have the right to raise a complaint directly with Tara Phillips. Complaints can be made by email, via the website contact form, or in writing. Data protection complaints will be acknowledged within 30 days.
If you remain dissatisfied, you have the right to contact the Information Commissioner’s Office (ICO).
