At Tara Phillips Hypnotherapy, I take your privacy and the protection of your personal information seriously.
This page explains how you can raise a concern or complaint about how your personal information has been collected, stored, processed, shared or otherwise handled.
This complaints process is for data protection and privacy concerns only. It is not intended for general complaints about services, appointments, treatment outcomes or other non-data related matters.
What is a data protection complaint?
You may use this process if you have a concern about how your personal information has been handled. This may include concerns about:
- how your personal information has been collected;
- how your personal information has been stored or protected;
- how your personal information has been used;
- how your personal information has been shared;
- the accuracy of personal information held about you;
- how a request relating to your personal information has been handled.
Complaints relating to therapy services, professional conduct or treatment outcomes should be directed through the appropriate professional complaints process.
How to make a complaint
You can submit a data protection complaint electronically using the form on this page.
Please include your name, email address and details of your concern so that I can understand and respond to your complaint.
What happens after you submit a complaint?
I will acknowledge receipt of your data protection complaint within 30 days.
Your complaint will then be reviewed and appropriate steps will be taken to investigate the concern without undue delay. I may contact you if further information is needed.
Once the complaint has been reviewed, I will respond with the outcome.
Escalating a complaint to the ICO
If you remain dissatisfied following the outcome of your complaint, you have the right to raise the matter with the Information Commissioner’s Office, which is the UK regulator for data protection matters.
The ICO recommends raising concerns with them within three months of your last meaningful contact with the organisation about the issue.
You can find more information about making a complaint to the ICO on the ICO website:

Please use this form only for concerns relating to personal data and privacy.
